Services
Personalized support, built around one person.
Every client receives a personalized support plan. What that plan includes depends on who they are, what they want, and what they’re navigating right now.
How we help
Five kinds of support, in one relationship
-
Connect
Moving into a new community doesn’t automatically create a life there. We help clients find activities that actually interest them, meet people they genuinely enjoy, and become part of the place they live rather than a resident of it.
-
Navigate
New environment, new systems, new questions. We help clients find and access resources, services, transportation options, activities, and practical solutions to the everyday challenges that come up.
-
Plan
We work with clients to set personal goals and build routines that support independence and quality of life, then adjust as things change.
-
Advocate
Sometimes the hardest part is being heard. We help clients articulate what they need and navigate conversations with families, community staff, and service providers.
-
Reassure
With the client’s authorization, we keep families informed. Not surveillance. Communication, on terms the client agrees to.
In practice
What a plan can include
Depending on the client’s personalized plan, support may include:
- Scheduled in-person visits
- Regular check-ins
- Community integration support
- Activity and interest discovery
- Resource research
- Transportation and resource navigation
- Appointment and meeting coordination
- Advocacy and communication support
- Family communication, with client authorization
- Personalized goal setting
- Ongoing transition support
The goal isn’t to take over. The goal is to help the client remain independent.
